CHOOSING SELF-DETERMINATION
Services through the public mental health system begin with a determination of eligibility.
Eligibility is determined at least annually and may change as your needs change.
You decide where you want to live, work and have fun.
You determine who you want to help you reach your dreams and goals.
Your family, friends and chosen allies can help you with decisions to make sure your health
and safety needs are met.
You will have an Individual Service Budget. It is your responsibility to stay within your budget
and spend money more efficiently.
Self-determination arrangements must be financially responsible and cannot cost more
than traditional services.
If you are eligible for public mental health services you may receive those services in
the following ways:
- Traditional service approaches
- Self-determination using KCMHSAS providers, other providers or employees you hire
- Self-determination using other community resources
WHAT SHOULD I DO?
If you are interested in self-determination you would start by discussing this option with your supports coordinator, case manager, or KCMHSAS self-determination staff.
WHY SELF-DETERMINATION
Self-determination is a different way of using the Medicaid funds you are eligible for.
- Freedom to plan your life with supports necessary for that life
- Authority to control a set sum of dollars in order to purchase supports
- Responsibility of accepting a valued role in your community and of being accountable for using public dollars in a responsible way that is consistent with Medicaid requirements
- Support in arranging resources and people (both paid and unpaid) to assist you in living in your community
- Confirmation that people with disabilities, along with their family, friends and chosen allies, have the right to be a part of the decision-making that affects their lives.
REMEMBER
Self-determination is a method of using the Medicaid funds you are eligible for in a different way.
Consider creative options for your services, including natural supports, to help you be healthy and safe.
Self-determination emphasizes participation and personal control for individuals served through the public mental health system.
You can use a combination of self-determined arrangements and traditional services.
STEPS TO SELF-DETERMINATION
The self-determination movement is founded on being able to make decisions about your own life.
The steps to self-determination using Medicaid dollars:
- Begin by selecting this option as part of your person-centered-plan
- Determine how you will use Medicaid dollars to reach your goals
- Work with your supports coordinator, case manager or KCMHSAS self determination staff to determine the level of responsibility you want to assume
- Complete the required paperwork
